CPT® Code 17003 is used to report the destruction of premalignant lesions for the second through fourteenth lesions treated during the same encounter. This add-on code is most commonly used alongside CPT 17000, which covers the destruction of the first premalignant lesion.

Premalignant lesions, such as actinic keratoses, are often treated using methods including cryotherapy, electrosurgery, laser surgery, chemical treatment, or curettement to prevent progression to skin cancer.

Important: CPT 17003 is an add-on code and cannot be billed alone. It must be reported in conjunction with CPT 17000.

To support CPT 17003 billing, documentation should include:

  • Patient identifiers and date of service
  • Diagnosis supporting treatment of premalignant lesions
  • Number of lesions treated
  • Anatomical locations of the lesions
  • Destruction method used (e.g., cryotherapy, electrosurgery, laser)
  • Medical necessity for treatment
  • Patient tolerance and post-procedure instructions
  • Provider signature and credentials

Reimbursement and Coding Considerations

  • CPT 17003 is an add-on code and must be billed with CPT 17000
  • Report one unit of 17003 for each additional lesion treated after the first, up to fourteen total lesions
  • For treatment of 15 or more premalignant lesions, report CPT 17004 instead
  • Diagnosis coding should support the presence of premalignant lesions, such as actinic keratoses
  • Documentation must clearly identify the number of lesions destroyed
