Pick an EMR

Choosing the right electronic medical record (EMR) and practice management system for your independent practice is a major decision that impacts your day-to-day operations, patient experience, and overall business growth. With so many options on the market, it’s easy to get overwhelmed. This guide walks you through the key steps to selecting an EMR that aligns with your current needs and future goals.

1. Define Your Business Needs

Before you start exploring EMR vendors, it’s essential to take a step back and clearly define what your practice requires. The better you understand your own needs, the easier it will be to identify the right system.

What are Your Business, Services, and Charting Needs?

Consider the core services you offer and how they will be documented and billed. Ask yourself:

  • What types of services will you offer? Are you providing traditional psychiatric care, offering functional medicine, medspa services, or incorporating wellness therapies? Each specialty has different charting, billing, and documentation needs. Or planning to offer multiple different types? In which case you need a flexible system.
  • Do you need access to pre-built or customizable templates? Functional medicine practices may want MSQ templates pre-built, plus the option to customize their own intake forms. Psychiatry practices will want to ensure they’re able to use the major scales built into the system (like PHQ-9, GAD-7).
  • Will you need inventory management? If you plan to offer injectables, supplements, or wellness products, you’ll need a system with built-in inventory tracking.
  • Will you do ePrescribing (eRx) or connect with labs? If so, make sure the EMR supports eRx and integrates with lab vendors.
  • What’s your monetization game plan?
    • Insurance-based? You'll need robust insurance billing features with eligibility checks.
    • Self-pay or hybrid? Features like packages, memberships, and gift card management become valuable for revenue diversification.

How Do You Expect Your Business to Change in the Next 6–18 Months?

Too often, we think about our immediate business goals - but you’ll want your EMR to be able to grow with your practice’s medium- and long-term goals. Think about your growth strategy:.

  • Might you expand services: Will you add new treatment modalities (e.g., ketamine therapy, hormone optimization)? Choose a scalable system with flexible charting capabilities.
  • Might you add providers and grow your team: Will you add additional providers? Offering the same or different service types? If you plan to add more providers or support staff, ensure the EMR offers multi-provider scheduling and customizable access levels.
  • Might you open additional locations: What would expansion look like for you? Consider whether the EMR offers multi-location support with centralized scheduling, inventory, and billing management.

2. Create Your Product Requirements

Once you’ve identified your business needs, translate them into specific EMR features and support requirements.

List the Services You Need

Identify the core features your EMR must include:

  • Clinical features: Customizable charting, templates, eRx, lab integrations.
  • Patient management: Scheduling, automated reminders, patient portal.
  • Billing and payments: Insurance claims, self-pay, integrated POS, and invoicing.
  • Inventory management: If applicable, tracking quantities, lot numbers, and expiration dates.
  • Business management: Time cards, office communication/chat functionality, task management, reporting and analytics.
  • Marketing and CRM tools: Automated follow-ups, text/email marketing, and lead management.

Determine Your Wrap-Around Support and Content Needs

Identify what other features are important to you:

  • Pre-built content: Like clinical templates, intake forms, or patient handouts to help you build your practice faster
  • Training resources and support: Check if the vendor offers live support, training libraries, and onboarding assistance.

Map Out Your "Must-Have" vs. "Nice-to-Have" Features

Not every feature is essential - and there’ll be a trade-off between pricing and functionality. Decide ahead of time whether your features are:

  • Must-have: Core features critical to your practice’s operations that you cannot do without.
  • Nice-to-have: Features that would be helpful but not essential (e.g., advanced marketing integrations, aesthetic customization).

3. Do Your Competitive Research

Once you know what you’re looking for, it’s time to explore the market.

Generate a List of Top Vendors

  • Use a mix of sources:
    • EMR review sites like Capterra, G2, and Software Advice.
    • Peer recommendations from colleagues or professional groups.
    • Specialized forums or social media groups for independent providers.

Preliminary Research: X Out Unfit Vendors

Eliminate vendors that don’t meet your minimum requirements:

  • Limited customization? Not a fit.
  • No eRx or lab integration? Cross them off.
  • Lacks integrated billing? Move on.
  • Can’t support multi-location or growing practices? Skip them.

Schedule Demos with Your Top 3 Vendors

Get hands-on experience with the systems by scheduling demos. EMRs include so much functionality, it’s helpful to view online videos and come with your questions ready to go.

  • View an initial video ahead of time and come prepared with your questions
  • Ask the sales team to walk you through use cases specific to your practice.
  • Take note of the user experience, ease of navigation, and flexibility.
  • Check the reporting capabilities—can you track revenue, patient metrics, and outcomes easily?

Play Around with the System

If the EMR offers a free trial or sandbox account, take advantage of it. If not, ask why not.

  • Explore the knowledge base and support resources.
  • Create test patients and practice charting and billing.
  • Explore the patient portal from both the provider and patient perspectives.
  • Test key workflows like scheduling, charting, and payments.

4. Keep an Eye on Contractual Requirements

Think you’re ready to sign on the dotted line - before you get too excited, scrutinize the fine print.

Check the Contract Before Committing

  • Avoid annual contracts if you’re still refining your business model. Go for month-to-month contracts until you’re confident the system meets your needs.
  • Understand early termination clauses and hidden fees.

Know the Data Export Requirements

  • Ask about data ownership—can you easily export your data if you switch EMRs?
  • Find out if there are fees for data exports or ongoing read-only access plans.
  • Check if the EMR offers data storage options after termination to maintain compliance with record-keeping regulations.

5. Plan Your Transition

Even the most intuitive EMR requires setup and familiarization; once you’re signed up, create a manageable and realistic plan for your go-live.

Create a Realistic Timeline

  • Expect setup and data migration to take 2–6 weeks, depending on the complexity of your clinic.
  • Factor in time for staff training and practice runs.

Gather Your Documentation

  • Prepare essential documents: patient forms, consent templates, service descriptions, and existing patient data.
  • If switching from another EMR, arrange for data migration.

Sign Up and Set Up Your Account

  • Customize your EMR settings to match your workflows.
  • Add branding elements like your logo and business info.
  • Configure your charting templates, billing rules, and notifications.
  • Think through your patient communications and email/text tone and style.

Do a System Check-Up

Before going live, conduct a thorough system review:

  • Test patient scheduling, charting, and payments.
  • Verify email and SMS notifications are configured correctly.
  • Ensure billing and insurance features are working properly.

Practice Before You Go Live
If there are multiple folks in your clinic, or support team members make sure everyone does this step!

  • Use test patients, family members, or staff (we’ve even seen folks practice on and diagnose their pets - typical diagnosis of hyperactivity, and houseplants - dehydration is a common issue there) to simulate real scenarios.
  • Go through a full visit workflow: scheduling, intake, charting, and taking payment.
  • Iron out any kinks before seeing real patients.

Schedule Your First Patient

Once you’re confident with the system, you’re ready to see your first patient.
