Thank you for choosing Harmony Health and Mental Wellness for your mental health care. We are pleased to assist you on your journey toward wellness. Your well-being is our priority, and we are committed to providing compassionate and personalized care.

You can easily schedule your first appointment online at your convenience. If you have any questions or need assistance with the scheduling process, please don't hesitate to reach out. Please be mindful that all appointments must be confirmed by the provider. Once your appointment is confirmed, you will receive an email prompting you to sign up for the patient portal. Once you have logged into the portal, please complete all intake forms. Forms must be completed 24 hours prior to your appointment to prevent cancellation.  

Once you've signed into your Patient Portal, be sure to upload a copy of your drivers license and the front and back of your insurance card. You can do this in the "Documents & Handouts" section of the portal, which you will see on the left side navigation bar. Select Record Type from the drop down (ID Card or Insurance Card) and choose the file to be uploaded. Select Upload file.
