If your need is urgent or emergent, please seek care immediately at the nearest Urgent Care or Emergency Department. Also, please note that you must have either an initial new patient appointment (if you are a new patient to Angel), or a "Moving Over" visit if you are a prior patient of Angel's to establish care prior to booking an acute visit.
You may use the "Moving Over 60 minute" visit if you have a medical need at the time of your first visit if you are a previous patient of Angel's wanting to continue care with her. This visit is for new patients to Angel. It is a 90-minute appointment to establish care, review lab results, complete a physical exam, and collaboratively develop a personalized health plan. It is best to get your lab panel (bloodwork) done 2 weeks prior to the time you schedule for an appointment so that it can be reviewed at the time of visit. After booking, we will be in contact to get the labs ordered for you.
*** The preferred lab panel is the 'Full Lab Panel'. If you are undergoing hormone replacement therapy or need hormone levels checked, please also select the hormone panel. You will be contacted about getting your labs before you visit CPL (this is the lab we have a contract with to get best pricing). Please allow time for us to input your lab order into the system. We will send you instructions and get authorization to charge your card prior to having your labs drawn. ***
Pediatrics from birth to 18 years who are new patients to Angel OR for those that have seen her but are HAVING ANY ISSUES THAT NEED ADDRESSED. This visit is 45 minutes to allow for problem-solving and developing a plan of care to address any acute or on going issues.
This is the initial visit for pediatric patients (up to age 18) transferring care from Veritas that have been seen in person by Angel within the past 3 to 4 months and simply need to establish care (no new / acute issues at hand). If you have issues that need to be addressed, please choose the "follow up visit" option.
For pediatrics up to age 18 for regular screenings, assessments, and wellness checks. If you are needing a sports or school physical, this is the visit to book.
This visit will be booked for an annual exam, lab review, and to update your plan for overall wellness. Labs will need to be completed 7-10 days before your appointment in order to review labs and adjust the plan of care accordingly at your visit.

Schedule this visit for a one hour "following up" on your progress visit. This visit is not the same as an annual visit but is available for when a visit and care is needed between annual visit times. *It is best to get your lab panel (bloodwork) done 7-10 days prior to the time you schedule for an appointment so that it can be reviewed at the time of visit if it has been requested by your provider or you may message Whitney with questions if you feel there may be some labs needed prior to your appointment.

This visit may be scheduled only if you have had a face-to face exam with your practitioner within the past year. It is a one hour Telehealth visit to address ongoing medical issues for those who are unable to come into office for a face-to-face visit.
This is a 15-minute visit for evaluation of an acute illness, such as a sore throat, cough, cold, urinary tract infection, or acute diarrhea. This visit is intended for straightforward cases and does not include complex abdominal pain, chronic gastrointestinal issues, or other conditions requiring extensive medical decision-making. If you are experiencing abdominal pain or need evaluation for chronic issues, please note that you will be scheduled for a 30-minute case review or a 1-hour follow-up visit.
This is a telehealth 15-minute visit for evaluation of an acute illness, such as a sore throat, cough, cold, urinary tract infection, or acute diarrhea. This is the initial visit (60 min) to FHHC for patients transferring care from Veritas who were already seeing Angel, that have NOT been seen in the last 3-4 months and/or have issues that need to be addressed/ needing a full appointment time. After you schedule this appointment, we will contact you to see what labs may need to be done prior to this appointment or you may message Whitney in the portal with any questions on labs/ your ?upcoming visit.
This is the initial visit to FHHC for patients transferring care from Veritas that have been seen in person within the past 3 to 4 months who need to establish care via telehealth and NEED TO ADDRESS AN OLD, NEW, OR ACUTE ISSUE. If your visit will require an update in your treatment, this is the visit you need to book if you are a prior patient of Angel's.
**This visit is available for scheduling as deemed necessary by provider only when a full one hour appointment is not neccessary**
A case review is a 30-minute visit, designed for 'check-in' appointments with your practitioner to make minor adjustments to your care plan or to address more complex acute concerns, such as abdominal pain that requires more time than a standard acute visit. These appointments are scheduled at the discretion of your practitioner; otherwise, a one-hour follow-up visit will be arranged.
