By Scheduling online, you acknowledge and agree to the following cancelation policy :

ALL patients will be required to have a credit card on file.

• NO CALL/NO SHOW for an appointment for all patients will result in a $100 fee.
• Cancelations or reschedules with less than 24-hour notice will be charged a non-refundable and non-transferable fee based on the appointment that is canceled.
• Patients that are more than 10 minutes late will not be able to be seen and late fee will be charged.
