This is the first consultation to discuss hormone replacement therapy.  This visit includes going over medical history, review, if no labs, will require a lab draw in the office or lab order to obtain needed hormone levels for therapy.  DIscuss benefits of HRT along with side effects.  If you are a new patient already on pellets and have your dosing or recent labs, please schedule pellet appointment.  (Please note that hormone consult price does not include labs )

This initial consult will include seeing one of our Nurse Practitioners to review your medical history and discuss our weight loss options.  If required labs are needed this will be an additional cost. 

This visit will be with one of our RNs.  At this visit we will obtain your weight, review dosing, side effects, and diet/exercise.  Will determine if your dose needs to be increased, decreased or remain the same.  Any recommendations to relieve side effects or to increase weight loss will be addressed.  We can order your next vial if needed.  Order any labs if needed. 

Must be an established patient.  If not, you must make a new patient appointment with one of our Nurse Practitioners. 

This visit will be with one of our RNs.  At this visit we will obtain your weight, review dosing, side effects, and diet/exercise.  Will determine if your dose needs to be increased, decreased or remain the same.  Any recommendations to relieve side effects or to increase weight loss will be addressed.  We can order your next vial if needed.  Order any labs if needed. 

Must be an established patient with recent vitamin D blood work.  If not an established patient, you must make a medical visit appointment.  This injection requires a vitamin D level before any treatment can be given, and you have to meet with one of our Nurse Practitioners.

This is for patients who have completed the 6-week initial treatment and now are on maintenance therapy.

This treatment is for stress incontinence.  It requires no numbing and is 30 minutes in duration.  It consists of treatment once-a week for 6 weeks for optimal results.
