During this 30 minute telehealth/virtual visit, we will discuss your medical history, weight loss goals, and how the program works.  If you have had recent lab work completed, please be sure to have that available for this appointment. 
This visit includes information about weight loss injections: GLP1, Mega Burn, MIC + B12, and/or oral medication: bupropion/naltrexone. 

Follow up/check-in visit where we will discuss pounds and/or inches lost, side effects, and any questions or concerns that you may have. Medications may be reordered at this time if needed. 

Consultation: We will discuss your concerns and the labs that you are requesting.  

You will have the option to: 
1. BILL INSURANCE: Obtain a lab form to take to your lab of choice and they will bill your insurance.  This can sometimes be more expensive than cash pay, dependent on any co-pays or the deductible that you have with your insurance company.  You are responsible for any outstanding charges not covered by your insurance company.  
2. CASH PAY:  You can get your labs drawn at any LabCorp location. You will pay Vitality Health and Wellness, LLC the cost of the labs, which vary depending on what is ordered, and you will be responsible to pay the lab a $10 draw fee.  

Once testing is complete and the lab results have been obtained by Vitality Health and Wellness, LLC, a follow up visit will be conducted to review the results and recommendations will be given at that time.
