New patient consultation visit to go over past medical history, diet/exercise, physical examination and develop a plan of care. 

Visit is available for telemedicine if not interested in a controlled substance. 

The charge for this visit does not include any medication. If labwork is recommended by practitioner, there will be an additional fee for labs which will be discussed prior to being performed.

Labs will be performed at this visit so please let provider know if you are not fasting as it will affect the results of certain tests. Please drink at least 1 bottle of water prior to your visit time. 

The lab will only accept bloodwork on Monday - Thursday. If you come on a Thursday after 3pm - Sunday, you will need to come in on a separate visit for labs.
