Members pay a monthly or annual fee to the medical practice to have unlimited access to certain primary care services as needed, without paying an additional fee at the time of service. Most routine screenings, management of chronic conditions, and acute-care visits are covered in this type of practice. 

For acute or chronic conditions. A $15, nonrefundable booking fee is required to secure your appointment time- this will be applied towards your appointment fee. If you need to reschedule, this will be credited to your account and will be applied to your future appointment.
