CANCELLATION POLICY: We require at least 24 hours notice for all cancellations/reschedules so that we can keep our scheduling system running smoothly. Cancellations with less than 24 hours notice are subject to a $100 missed appointment fee. Please refer to the Quantum Patient Policy Agreement in your Welcome Email for more information on our financial and organizational policies. Thank you for your understanding!

New Patient $ 375.00

For new patients age 18-64 to establish care with our office.

For new patients under 18 or 65+, we offer a discounted new patient rate. Please let us know your age under "reason for visit" when scheduling.

60 min Follow Up $ 250.00

For established patients with:

New concerns
Hormone therapy management
Discussion of perimenopause/menopause, pelvic pain, or sexual dysfunction
3 or more medications/treatments
Pelvic exams, physical exams, biopsies, IUD
Annual visits

60 min Follow Up - Telehealth $ 250.00

30 min Follow Up (LIMITED) $ 180.00

For established patients with:

1-2 medication refills (no new concerns)
Reviewing lab-work
Weight management
Urgent visits (UTI, vaginitis, side effects)

30 min Follow Up (LIMITED) - Telehealth $ 180.00
