Scheduling & Payment Policy
  • At Freeman Wellness and Aesthetics, we require full payment for the visit upfront at the time of scheduling the appointment.
  • Patients can change their appointment date and time up to 24 hours before the scheduled appointment.
  • After the 24-hour mark, rescheduling will no longer be possible, and the payment will be forfeited.
  • Patients who are more than 15 minutes late for their scheduled visit will have their appointment considered canceled and will forfeit their payment.
By scheduling and paying for this visit you are agreeing that you understand the above statements and will adhere to them.

Weight Loss & Management - Initial Visit 45 Min $ 140.00
This visit is to establish care, discuss your weight loss goals, obtain a detailed health history, obtain labs, discuss treatment/medications options. This appointment will include a 1-2 week follow up phone call to review labs and discuss medications (This follow-up is included in the $140.00 price) 
Weight Loss & Management - First Month Follow-Up 20 Min $ 50.00
Patients will be seen 4 weeks after the start of medications to discuss medication satisfaction, dose adjustments, questions/concerns, and obtain medication refills.


*After this visit patients will be seen every 3 months or more frequent if needed or requested. 
Weight Loss & Management - Follow-Up Visit 30 Min $ 100.00
Patients will be seen every 1-3 months for follow-up to evaluate weight loss, discuss medication satisfaction, dose adjustments, questions/concerns, and obtain medication refills.


Patients must be seen at least every 3 months. 


*More frequent visits may be necessary initially until a desired dose/regimen is established.
Primary Care - Initial Visit 45 Min $ 100.00
Patients wishing to establish care for routine primary care visits, medication management, chronic disease management, labs, tests/imaging.
(Visit time: 45 minutes - 1 hour)
Primary Care -Follow-Up Visit with Medication Refills 25 Min $ 50.00
Routine follow-up visits for established patients & medication refills. 
(Visit time: 20 - 30 minutes) 
*Patients who have not been seen in greater than 13 months will require a new initial visit. 
Sick Visit - Established Patient 30 Min $ 50.00
Patients who have established care with Freeman Wellness & Aesthetics in the past and have been seen within 1 year.
(Visit time: 20 - 30 minutes) 
Sick Visit for New Patient 30 Min $ 75.00
Patients who have not established care with Freeman Wellness & Aesthetics. (Visit time: 20 - 30 minutes) 
Follow-up Sick Visit 20 Min $ 30.00
Established or Non-Established patient who have been seen within 1-14 days for a past sick visit with our office. (Visit time: 15 - 25 minutes) 
Men's Health - Initial Visit 45 Min $ 140.00
Initial visit to Establish care, review medical history, symptoms, order labs, discuss treatment options. This appointment will include a 1-2 week follow up phone call to review labs and discuss medications (This follow-up is included in the $140.00 price) 
Men's Health - First Month Follow up Visit 30 Min $ 50.00
Patients will be seen 4 weeks after the start of medications to discuss medication satisfaction, dose adjustments, questions/concerns, and obtain medication refills.*After this visit patients will be seen every 3 months or before if needed or requested. 
Men's Health - Follow up Visits 20 Min $ 100.00
Established patients will be seen every 3 months (or more frequently) to discuss treatment results obtain lab work and medication refills. 
Patients must be seen at least every 3 months.
*More frequent visits may be necessary initially until a desired dose/regimen is established.
Mental Health - Initial Visit 45 Min $ 100.00
Patients will be seen 4 weeks after the start of medications to discuss medication satisfaction, dose adjustments, questions/concerns, and obtain medication refills.

*After this visit patients will be seen every 3 months or before if needed or requested.
Mental Health - First Month Medication Follow-Up 20 Min $ 50.00
Patients will be seen 4 weeks after the start of medications to discuss medication satisfaction, dose adjustments, questions/concerns, and obtain medication refills.
Mental Health - Follow up Visits 40 Min $ 75.00
Established patients will be seen every 3 months to discuss treatment results obtain lab work and medication refills. 

*More Frequent Visits may be required initially until a desired treatment is identified.
