This is the first consultation for all patients. Includes full physical exam, medication review, and prescription refills.
General physical is a screening exam for ages 6 months to 18 years that includes a review of past medical history and medications, a vaccine review, as well as measurement of vital signs: height, weight, body mass index (BMI), blood pressure, temperature, and pulse. This physical exam includes prescription refills in addition to patient education related to acute and chronic diseases.

This physical exam includes prescription refills in addition to patient education related to acute and chronic diseases.

This physical exam includes prescription refills in addition to patient education related to acute and chronic diseases.

General physical is a screening exam for adults ages 65 years and older that includes a review of past medical history and medications, a vaccine review, as well as measurement of vital signs: height, weight, body mass index (BMI), blood pressure, temperature, and pulse. This physical exam includes prescription refills in addition to patient education related to acute and chronic diseases.

All pre-op risk assessment includes a detailed health exam. All patients must provide pre-op risk assessment paperwork if completing cosmetic surgery.
Provide telehealth services via computer for all non-acute home health care needs. 
Screening for kids who desire to participate in sports. Includes review of medical and family history, vitals, height and weight, vision screening, focused assessment, and paperwork completion.
One-time visit that will allow the review of medications and prescription refills for non-established patients.
Collect labs at home and call with updated results within 72 hours. 

Complete Blood Count: Measure different parts of your blood. Should be considered when there is new onset fever, chills, fatigue, malaise, or in preparation for operative intervention. 
Complete Metabolic Panel: Overall assessment of liver, kidneys, and electrolytes.
Lipid Panel:  Determines risk for cardiovascular disease.
Liver Function Test:  Check for the overall health of your liver. 




Collect and monitor CBC. Will call with results within 72 hours. 
Measure different parts of your blood. Should be considered when there is new onset fever, chills, fatigue, malaise, or in preparation for operative intervention.
