New patients please book here. This appointment will be conducted through our virtual Telehealth platform, you will receive a link in your confirmation email that you will need to click on to access your visit and the appointment date and time.

If you prefer an in-person, please book exit this booking and book under our in-person option

You can expect this appointment to last about one hour. During this time, the provider will get a better understanding of who you are as an individual. This includes reviewing your history, learning about your current symptoms and establishing a treatment plan.

Upon booking, you will receive an email confirming your appointment. In this email you will find several electronic forms that are required to be completed before your appointment. Incomplete forms will result in cancellation of your appointment. 

Patients using insurance, be sure to enter accurate and complete insurance details when booking this visit. Insurance verification takes about 48 business hours. 


The $75 fee is a deposit, should you be using your insurance for your visits, this deposit will be refunded to you at your first visit. If you miss the appointment or cancel within 24 hours of your appointment time, this $75 fee will be forfeited
