Schedule this appointment if you are new to our practice and you would like to learn more about the medical weight loss programs
Cost:
Please note the following regarding our weight loss program:
$100 consultation fee is due at the time of scheduling the appointment and will not be billed to your insurance
If you choose a weight loss program (excluding prescription brands covered by insurance), I will credit you $50 towards the first month's plan
Medication costs are not included in the consultation fee.
We will complete any necessary prior authorizations with your insurance company for weight loss medication, if applicable.
If medication is approved with your insurance, I charge $20 per month to refill medication (After 1st month). Invoice due prior to reordering.

Once scheduled, an email will be sent to you with a link for the patient portal to complete required documents as well as the link for virtual visit.

All consent forms need to be signed and intake forms need to be completed.

Schedule this visit if you are an established weight loss patient and require monthly prescription refills. These brief reviews will cover your progress, updates and any necessary changes to treatment plan. A $20 charge will be required to book appointment and refill prescription.
Schedule this appointment if you are new to our practice and need care for chronic conditions such as diabetes or hypertension. This visit includes a comprehensive assessment, necessary lab orders, and prescription management.
Cost:
$100 for self-paying patients (includes one follow-up visit)
Co-pay only if using insurance
Note:
We do not accept self-pay patients on Medicaid or Medicare. Insurance must be processed, and co-pays will be collected if applicable.
If we accept your insurance you will need to pay the $100 and then if co-pay is less than $100 you will be reimbursed
