Initial Appointment

Schedule this appointment if you are new to our practice and you would like to learn more about the medical weight loss programs

Cost:
  • Please note the following regarding our weight loss program:
    • $100 consultation fee is due at the time of scheduling the appointment and will not be billed to your insurance
    • If you choose a weight loss program (excluding prescription brands covered by insurance), I will credit you $50 towards the first month's plan
    • Medication costs are not included in the consultation fee.
    • We will complete any necessary prior authorizations with your insurance company for weight loss medication, if applicable.
    • If medication is approved with your insurance, I charge $20 per month to refill medication (After 1st month). Invoice due prior to reordering. 
Note:

  • Once scheduled, an email will be sent to you with a link for the patient portal to complete required documents as well as the link for virtual visit.

  • Please ensure all documents are completed on the portal before your visit begins to avoid any delays.
  • All consent forms need to be signed and intake forms need to be completed.

Schedule this visit if you are an established weight loss patient and require monthly prescription refills. These brief reviews will cover your progress, updates and any necessary changes to treatment plan. A $20 charge will be required to book appointment and refill prescription. 
Initial Appointment

Schedule this appointment if you are new to our practice and need care for chronic conditions such as diabetes or hypertension. This visit includes a comprehensive assessment, necessary lab orders, and prescription management.

Cost:
  • $100 for self-paying patients (includes one follow-up visit)
  • Co-pay only if using insurance
Note:
  • We do not accept self-pay patients on Medicaid or Medicare. Insurance must be processed, and co-pays will be collected if applicable.
  • If we accept your insurance you will need to pay the $100 and then if co-pay is less than $100 you will be reimbursed
  • Once scheduled, an email will be sent to you with a link for the patient portal to complete required documents as well as the link for virtual visit. Check SPAM folder for email.
  • Please ensure all documents are completed on the portal before your visit begins to avoid any delays.
  • All consent forms need to be signed and intake forms need to be completed.

Schedule this appointment if you are new to our practice and experiencing a minor sudden illness such as a cold, UTI, flu, or rash.

Cost:
  • $50 collected at time of appointment
  • If insurance is accepted, any co-pay or co-insurance amounts will be required. If these amounts are less than $50, they will be reimbursed.

  • You will receive an email with a link to create an account on the portal. Please fill out all forms prior to the visit. CHECK YOUR SPAM FOLDER
  • We do not accept self-pay patients with Medicaid or Medicare. Schedule an Urgent Care Visit with insurance.

Schedule this visit if you are an established patient experiencing a sudden minor illness.

Cost:
  • $35 collected at time of appointment
  • If insurance is accepted, any co-pay or co-insurance amounts will be required. If these amounts are less than $35, they will be reimbursed.

Note:
  • We do not accept self-pay patients on Medicaid or Medicare. Insurance must be processed, and co-pays will be collected if applicable.
  • Please complete acute care intake form prior to visit.

Prescription Refill Appointment

Schedule this appointment if you need a prescription refill, and I am your primary care provider, or if you can't reach your provider.
Important:
  • No narcotics, controlled substances or benzodiazepines will be prescribed.
  • You must show your current prescription bottle for verification during the virtual visit.
  • Do not book this appointment for any new medication. Instead, schedule an urgent care visit or a new patient primary care appointment.
Follow-Up Appointment

Schedule this appointment ONLY if you are an ESTABLISHED patient needing continued management of your chronic condition.
