Please bring a list of medications with you and any clearance letters and/or testing results you may need from a specialist. If you have a CPAP/Sleep Apnea please bring a printed copy of your compliance report. 


Please drink water as a urine specimen will be obtained on day of exam as wel as a vision screening (bring/wear any contacts or glasses) as well as hearing aides.

An electrocardiogram records the electrical signals in the heart. It's a common and painless test used to quickly detect heart problems and monitor the heart's health.

An electrocardiogram is a painless, noninvasive way to help diagnose many common heart problems. A health care provider might use an electrocardiogram to determine or detect:

Irregular heart rhythms (arrhythmias)
If blocked or narrowed arteries in the heart (coronary artery disease) are causing chest pain or a heart attack
Whether you have had a previous heart attack
How well certain heart disease treatments, such as a pacemaker, are working
You may need an ECG if you have any of the following signs and symptoms:

Chest pain
Dizziness, lightheadedness or confusion
Heart palpitations
Rapid pulse
Shortness of breath
Weakness, fatigue or a decline in ability to exercise
At this visit will discuss holistic/natural approach to health that could include supplements and tests that are not traditionally done in Western/Traditional Medicine. 

This type of approach can be for any type of illness from acute/sick issues to chronic disease management (diabetes, mental health, etc). 

Some laboratory tests might be covered by insurance. Can discuss at time of visit.

Pediatric visit not listed on any of the other services. Please select for a problem visit that is not a sick visit. Examples include: Asthma flare, rash, laceration/cut, diabetes visit, etc. If you have any questions please contact us. 
If this is an emergency or needs emergent care please call office to discuss or call 911 or go to local Emergency Room. If you are needing sutures/stitches due to cutting yourself, please call the office immediately to discuss as some lacerations/cuts can not be repaired in office and require a Emergency Room visit.

Coronavirus Disease 2019 (COVID-19) | CDC

We offer in office rapid Covid 19 testing and send out PCR testing. 

Other in office tests include: Influenza A &B, RSV, Urine dip, Urine pregnancy, Strep throat. Tests take about 15 minutes and will get results immediately along with a treatment plan. 

Can also do Sick visits via Video/Telehealth as long as they can be safely done via that method. Please call the office to discuss this if needed video visit due illness/sickness. 
Will discuss symptoms during the visit.  Blood and urine will be collected during the visit, depending on symptoms. Please don't go to the bathroom at least 1 hour before collecting urine so the results are not a false negative.
