This is the first office visit with Jennifer Sadeck, NP to discuss a personalized weight loss plan. This takes place in-person at 519 American Legion Hwy, Unit 4. The visit typically includes the following:
Introduction & Patient History:
Overview of the patient's medical history, including any pre-existing conditions (e.g., diabetes, hypertension, etc.).
Discussion of previous weight loss attempts and challenges.
Review of medications currently being taken, including any related to weight or metabolism.
Comprehensive Weight Loss Assessment:
Measurement of current weight, body mass index (BMI), and other relevant health metrics (e.g., waist circumference, blood pressure, etc.) with use of video
Review of any recent visits with primary care physician to assess if any lab work or medical tests have recently been performed and will need to be requested to review. Assess if additional blood work or cardiac testing (EKG, stress testing) should be acquired prior to starting the program.
Health and Lifestyle Review:
Detailed discussion about the patient’s current diet, eating habits, and meal timing.
Assessment of current physical activity levels and exercise routine.
Discussion of sleep patterns, stress levels, and any factors affecting weight loss, such as emotional eating.
Review of Weight Loss Goals:
Collaborative setting of realistic short-term and long-term weight loss goals.
Discussion of motivation and barriers to success.
Medication Review & Recommendations:
Introduction to potential weight loss medications, such as Semaglutide or other treatments.
Explanation of how medications work, including potential benefits and side effects.
Discussion which route of obtaining weight loss medications will be done whether through health insurance versus compounded pharmacies.

Assess medication tolerance and determine if a dosage adjustment is necessary

This follow-up appointment is designed for patients who are already established with Medical Weight Loss Solutions and have begun treatment with compounded Semaglutide or alternative medicines (Sermorelin, Aminophylline/Glycyrrhetinic Acid Cream, etc), after their initial consultation.
During this session, we will:
