Please arrive to you appointment on time, if you anticipate difficulties with the video link, please arrive 10 to 15 minutes prior to your scheduled appointment. We do not provide the following services: Evaluations for firearms, Long term disability, Clearances to drive a truck, Active suicidal thoughts, Active Eating Disorder (Anorexia Nervosa), Postpartum Psychosis, Evaluation for Autism spectrum, and do not treat patients with Traumatic brain injury, Schizophrenia, Non-verbal learning disorders, and a history of complex polypharmacy medications. The following medications may be prescribed on rare circumstances on a case by case basis: Alprazolam (Xanax), Clonazepam (Klonopin), Diazepam (Klonopin) Diazepam (Valium) Lorazepam (Ativan).
We are currently not accepting any Medicaid patients for therapy. We are only seeing Aetna Better Health and Horizon NJ Health patients for psychiatric evaluation and medication management only.

I agree to enter a valid debit/credit card information to book an appointment with Pathway to Mentality. If my card information is incorrectly invalid, my appointment will be automatically canceled. If I no show or do not cancel my appointment 24 hours prior to my scheduled appointment, I agree that my card will be charged $50 for a new patient psychiatric evaluation appointment and $35 for a medication follow-up appointment in addition to a $2 credit card transaction fee. If I schedule a therapy assessment and my patient intake forms are not submitted within 24 hours of my scheduled appointment, your appointment will be canceled and charged a $50 late cancellation fee. This does not apply to NJ Medicaid patients, however if you no show to your first appointment, you will not be eligible in rescheduling your appointment.
The objective of a bariatric surgery mental health evaluation is to assess the patient's motivation, readiness, contraindications, support system, lifestyle, and mood symptoms prior to the patient having surgery. After the patient has the evaluation, the patient will either be cleared or not cleared in having bariatric surgery.
This evaluation is for new patients looking for therapy, a new therapist, and/or considering medication. During a biopsychosocial assessment, the therapist will assess biological, psychological, and social factor to better identify an individual's needs in coming up with a treatment plan.
Medication Management Follow-up for patients over age 18. A medication management appointment can take up to 30 minutes to complete. During a follow-up appointment, the clinician will assess your response to medication, assess side effects, mood, sleep, appetite, and provide microtherapy for your mental health concern.
Note: If your last appointment was six months ago, you would need to schedule a Psychiatric evaluation as opposed to a Medication Management Follow-up.
An annual cannabis renewal evaluation for NJ residents. Choose this option if your initial NJ Cannabis Evaluation was completed at Pathway to Mentality.
Initial Medical Marijuana Card Evaluation for NJ residents. The purpose of the evaluation is to determine if you qualify for one the conditions to obtain the medical marijuana card for the state of NJ. According to the NJ Cannabis Program, these are the following conditions: ALS, Anxiety, Cancer, Chronic pain, Dysmenorrhea, Glaucoma, Inflammatory Bowel Disease, including Crohn's Disease, Migraine, Intractable skeletal muscular spasticity, Multiple sclerosis, Muscular dystrophy, Opioid use disorder, HIV, AIDS, PTSD, Seizure disorder, including epilepsy, Tourette syndrome, and Terminal illness with prognosis of less then 12 months to live. Please provide evidence of past treatments you have tried for the condition you want to get the medical marijuana card for.
Initial Medical Marijuana Card Evaluation for NY residents. The cannabis evaluation will review your current medical conditions and assessed the reason for using cannabis. As of January 2022, there is no longer a list of quantifying conditions in the state of New York that a patient must meet to be certified for medical marijuana.

After the patient has the evaluation, the patient will either be cleared or not cleared to have the stimulator cord procedure.
