Thank you for booking an appointment with Indiana Testosterone. You will get an email that contained a link to complete the preregistration process. You will be required to complete your lab work and submit (fax) it to us before you can schedule an appointment. You will only be required to meet us in person once every six months. But, labs will likely be require more often than that. This is a subscription based service. This means you will be charged for an initial visit ($150) which will include your first month of medication and supplies. Every month after that will be $125 and it will include your medication and supplies (if needed).
Thank you for choosing Indiana Testosterone as your source for help with low testosterone! You will be emailed a list of lab work that needs to be completed before you will be seen. If the labs are not completed, or if we do not get the results before your appointment, the appointment will be reschedule or cancelled. These labs are not optional. We want to make sure you are a good candidate for testosterone replacement therapy. Please be aware that we only treat biological males 25 years of age and older with TRT. This is your follow up appointment. These are only required about every 3 to 6 months. We will review any needed lab work at this time. These appointments can be in-person or virtual. This is included in your subscription plan.
