 If you have been referred by a provider, we require a completed referral form. Please have your provider fax the form to us at 469-242-9713
This form helps us understand your current treatment plan and any specific concerns noted by your provider. After testing, we will send a detailed report of your results directly to your provider.

For self-referrals: You may request the Test without a referral, we will need to schedule an initial evaluation visit.This appointment is essential for gathering your mental health history and conduct ADHD specific assessments and evaluation.We will proceed with testing after the initial visit.
