Complete Forms Prior to Booking:
All required forms must be completed at least 48 hours before your appointment. If you need assistance, please contact us at 424-430-5025. Completing forms in advance allows your practitioner to familiarize themselves with your case, ensuring a more focused and personalized experience during your visit.
Punctuality and Late Arrivals:
Appointment times are strictly enforced. If you’re running late, please call us immediately. Arrivals more than 15 minutes late for initial visit and 10 minutes late for follow-up appointment will result in rescheduling and a late fee.
Confirm Your Appointment:
Appointments must be confirmed through the patient portal at least 24 hours prior. We will attempt to contact you 1-2 days before your appointment.
Medical Records for First Visit:
Please upload or bring any relevant records (e.g., lab tests and notes) from your prior doctor or nurse practitioner. This is important especially for people who are not familiar with all the treatments, procedures or medications they have taken so far. If you need us to request records, complete a medical release form and provide facility and nurse practitioner details. Allow at least 72 hours for records requests.
Medications and Supplements:
Bring all current medications, supplements, or a detailed list including name, dosage, frequency, and reason for use. This helps us provide a more thorough evaluation.
This is a comprehensive 60-minute evaluation for new patients. During this session, the practitioner will review your medical and psychiatric history, current symptoms, and concerns. A thorough diagnostic assessment will be conducted to develop a personalized treatment plan, which may include recommendations for therapy, medications, or lifestyle adjustments.
This session provides extended time for addressing more complex concerns or making significant changes to your treatment plan. It includes detailed assessments and collaborative decision-making for ongoing care.
