Booking Fee Policy: A non-refundable booking fee of $50 is required to secure your appointment. This fee can be applied toward the cost of a consultation plan if you choose to purchase one. However, if no plan is purchased, the $50 will be considered the cost of the consultation.

NOTE: NuLife Wellness & Weight Loss LLC is a cash pay practice. We do not bill insurance directly for services nor provide CPT codes for our services. However, we do provide you with a superbill for services rendered, available in your patient portal.
