How to prepare for your consultation:

• Consultation fee is collected at the time of your appointment.
• After you book your appointment, you will receive a confirmation email with the necessary questionnaires to complete.
• Complete all your intake forms and submit any previous lab results and you want your provider to review at least 24hrs prior to your scheduled appointment.

This is a comprehensive in-office consultation for patients interested in Functional Medicine to go over your symptoms, medical history, lab results, and treatment plan. The consultation fee includes a 20 mn follow-up virtual visit to review any lab results ordered during this consultation.
