Book this appointment if you are a new patient seeking an initial psychiatric evaluation. During this visit, a comprehensive assessment will be conducted to gather detailed information about your medical history, current symptoms, and psychosocial factors that may be contributing to your mental-health concerns.

This evaluation allows your provider to understand your specific needs, identify any underlying mental-health conditions, and determine the most effective course of treatment - which may include therapy, medication, or a combination of both.

Important: If you have questions about insurance coverage, please contact our office before scheduling. If we are out-of-network with your plan, the initial evaluation fee is $350.

After booking your appointment, you will receive an email containing intake and consent forms. Please complete all forms prior to your appointment. If you do not receive the email within 24 hours, contact our office immediately so we can resend it.

Cancellation Policy: Please provide at least 24 hours’ notice to cancel or reschedule.

Late cancellations or no-shows will be charged a $100 fee.

Book this visit if you are an established patient continuing your mental-health care.

During this session, we will review your progress, discuss any updates to symptoms or life changes, and adjust your treatment plan as needed. Follow-up visits may include medication management, supportive therapy, or both depending on your individual needs.
