Weight Loss Consultation

Book this appointment for your Initial Weight Loss Visit (NEW PATIENTS). This visit involves a comprehensive assessment of your health, lifestyle, and weight-related concerns. You may provide lab work from Primary Care Provider as long as it was completed within 6 months. 


Cost: Our weight loss initial consultation cost is $95. The $95 will be applied towards any Weight Loss service you pursue with us.

Weight Loss Follow-up

Book this follow up visit if you are an ESTABLISHED WEIGHT LOSS PATIENT with our clinic. 

 
If you have not yet had a weight loss consultation, please book a consultation first.  If you book this without having a consultation first, we will have to re-book your appointment.  

After booking this appointment, you will receive an email with a short follow up form to complete. Please complete this form prior to your follow-up visit. 
Shot Bar: Vitamin or Medication Injection
Book this visit to receive a vitamin shot or if you come to office for weekly medication injections.
General Consultation: Health and Wellness
Book this visit to discuss some options available for your overall health and wellness goals. 

Cost: Our general consultation for health and wellness cost is $95.

IV Therapy Visit
Book this visit to receive plain IV fluid or various additions based on patient symptoms.

Treatment plan to be determined with provider and patient.
IV Therapy - NAD+
Available Doses: 100mg, 250mg, 500mg.  Dose will be determined with your provider.  

Infusion time is dose dependent and may vary depending on patient tolerance.  
Hormone Replacement Therapy Consultation
Book this appointment for your Initial Hormone Replacement Therapy (HRT) Visit (NEW PATIENTS). 

Cost: Our Hormone Replacement Therapy (HRT) initial consultation cost is $95. The $95 will be applied towards any Hormone Replacement Therapy you pursue with us.
