Book this Appointment for an Initial Psychiatric Evaluation (New Patients Only). During this visit, a psychiatric evaluation (comprehensive assessment) will be conducted. We will gather detailed information about your medical history, current symptoms, psychosocial factors that may be contributing to your mental health concerns. This information allows the mental health provider to understand your specific needs, identify any underlying mental health conditions, and determine the most effective course of treatment, which may include therapy, medication, or a combination of both. 


After booking this appointment, you will receive an email with intake forms and consent forms.  It is very important you complete these steps PRIOR to your appointment. Please notify us immediately if you do not receive this email. 

$250 applies to non-insured patients. Co-pays will be discussed after insurance has been verified. 
Psychiatric Mental Health: ESTABLISHED PATIENT Visit 30 Min $ 125.00
Book this visit if you are an Established Patient to continue your mental healthcare.  
Minor Urgent Care 30 Min $ 75.00
Schedule this Appointment if you want to be seen as soon as possible for any of your Urgent Care needs. 
Minor Urgent Care: Medication Refill 15 Min $ 60.00

Book this Appointment for your Same day Prescription Refills (of established medications and non-scheduled only) until you are able to Follow-up with your primary care provider. 

Please have prescription bottles available for your appointment.

This visit includes:
-Lab review (Lab work required prior to initial visit)
-Discuss medication options. All medication and supplies will be shipped to your home. 
-Education on performing self-injections
