This is a 15-minute virtual/telehealth demonstration session for intramuscular (IM) or subcutaneous (Sub-Q) injections. Please ensure all your supplies are set up and ready, but do not open the alcohol pads to prevent them from drying out. If you have refrigerated medication, take it out at least 2 minute before the session begins. If you do not have a sharps container, use a heavy-duty, puncture-proof plastic bottle with a tight-fitting, screw-on lid as an alternative. Use this container for disposing of used syringes. Do not reuse syringes. Session will begin and end as scheduled.
Initial Personalized Lab Review, initial 1- on- 1 60-minute consultation with provider to review lab results and symptoms to discuss a customized performance and wellness protocol, first month of hormone therapy (E.g. pills, troches, patch, nasal gels, creams, vaginal inserts), 1 month of vitamin supplement, supplies to self-administer and medication. 

Pre-Appointment Requirements:
  • You will receive an email with required forms and consents to complete prior to your scheduled appointment.
Virtual Visits:
  • You will receive a text message with the link for your virtual visit on the day of your appointment. No application download is required.
  • You can attend the telemedicine session using any computer, tablet, or telephone that has a camera and microphone enabled.
Confirmation and Cancellation:
  • For all other services, you will receive a confirmation text one day before and on the day of your appointment.
  • If you need to cancel your appointment, kindly cancel at least 24 hours in advance.
Billing and Prescriptions:
  • Your credit card will be charged the total amount as listed.
  • Plans- Options 2-4: Pay in full or Half of total amount due at time of scheduling. Remaining balance due the following month.
  • Payment for Follow Up options will be charged as agreed upon between you and the provider.
  • Your provider will submit any prescriptions or lab orders following the visit, if indicated.

This is for MEMBERS ONLY who are enrolled in an Options Plan or Program by the clinic.

Monthly/Additional fees for Medication, Supplements and Add-ons 

Follow ups will need to occur every 2-3-months initially, allowing us to track your progress. Once you are at an optimal level of performance and wellness follow up visits can occur every 6 months. These appointments can be conducted via telehealth. Medication dosages will be adjusted according to your individual progress. Medication refills will be authorized according to follow up visits and lab results.
