This service is intended for the uninsured or those with high deductibles. 
Primary care should be accessible to everyone.
Primary care includes history and physical, medication management and prescribing.
Any Testing agreed upon by patient and provider will incur an additional charge.
Super bill can be provided for those who would like to submit it to their insurance.

The initial appointment includes consultation. History and physical. Goals and Discussion of Diet and Exercise Plan.
Please bring any current Labs to appointment.
Labs extra depending ?on insurance coverage.
Consultation fee will be deducted from monthly membership of $300 which includes coaching and medication.
