This is for patients who are already established and have paid for a weight management package. This is for an in office visit. Cost of medication is not included. 

This is for prospective patients that are not a part of the DPC program. This is for a prospective patient who wants to meet the provider and discuss options for weight loss services before making any commitments. This visit is not meant to be comprehensive and will instead just be a meet and greet with the provider as well as a brief run down of how the program works and options provided. A more comprehensive visit with a physical exam and measurements will occur for patients who have purchased a package. 

Schedule this if you're not an established Weight Loss patient. This is an in-office visit. This package includes an initial consultation with lab review or lab ordering if needed. It includes 3 months of accountability, weight ins, measurements. Package includes dietary counseling, supplement recommendations and a prescription for 3 months if warranted. It includes 3 in person or telehealth office visits. Cost of medication is not included.
