This is for an established patient who already has an active DPC membership. This is for patients who wish to do a follow up via telehealth only.
This is for patients who already have an active membership and have already had an initial visit. This will be a follow up visit. 
Patients age 6-17 years old who would like to enroll in a Direct Primary Care membership. Memberships require a minimum of a 6 month commitment and patients will receive 10% off for paying for 12 months upfront. Membership fees will be billed monthly. This is for patients who want to establish via telehealth and not have an in-person visit. 
Patients age 65 years old and above who would like to enroll in a Direct Primary Care membership. Memberships require a minimum of a 6 month commitment and patients will receive 10% off for paying for 12 months upfront. Membership fees will be billed monthly. We regret that we can NOT offer cash services for patients who have Medicare insurance plans, as it is illegal.
This is for a telehealth visit and not an in-office appointment. This service includes an in-person urgent care visit with the provider after standard business hours or on the weekend. Cost includes assessment and treatment plan by provider. If lab orders, referrals or prescription medications are given that will incur an additional cost.
