NEW PATIENT - Suitable for one or two chronic health problems or concerns. Includes a detailed evaluation of the patient’s social, past medical, and family history, a review of body systems, dietary evaluation, physical examination, assessment, and laboratory testing recommendations, as appropriate. This evaluation generally includes initial treatment, and requires approximately 45 minutes.
NEW PATIENT - Suitable for multiple chronic or complex health problems. Includes a detailed evaluation of the patient’s social, past medical, and family history, a review of body systems, dietary evaluation, physical examination, assessment, and laboratory testing recommendations, as appropriate. This evaluation generally includes initial treatment, and requires approximately 60 minutes. 
CURRENT PATIENT - Suitable for evaluation of treatment progress and slightly more complex new complaints. Includes thorough review of history, physical examination, referrals, and laboratory testing when appropriate. These visits generally require 25-30 minutes.
CURRENT PATIENT - Suitable for stalled treatment progress and need for fresh evaluation or new complex health issues not yet evaluated. Includes comprehensive review of history, physical examination, referrals, and laboratory testing when appropriate. These visits generally require 40-45 minutes.
CURRENT PATIENT - Suitable for evaluation of treatment progress and non-complex new complaints. Includes brief review of history, physical examination, referrals, and laboratory testing when appropriate. These visits generally require 15-20 minutes
CURRENT PATIENT - Suitable for assessment of an acute problem and immediate treatment or referral, as appropriate. May also be used for brief review of lab results or certain injections. These visits generally require 10-15 minutes
Yearly preventative medicine visit including a full review of systems, physical exam, labs, and recommendations to keep you feeling healthy! Codes and billing are according to age of the patient at the date of encounter, and the cost listed is for 99385. Possible CPT codes 99385, 99386, 99387
New Patient - Suitable for a single health concern. Includes review of relevant history, any necessary physical examination, assessment, and plan to address the complaint, including recommendations for appropriate testing and treatment options. These visits usually include initial treatment and are generally 30 minutes in length.
