Check in time is 15 minutes before your scheduled appointment. We require 48-hour notice if you are unable to keep your appointment. If you miss an appointment, fail to give sufficient notice, or arrive late for your check in, you will be charged $50 for that missed appointment.

New patients: Within the next 24 hours, you will receive a confirmation email with links to your new patient forms. Please fill out these forms as best you can before your appointment. Check your spam folder if you don't see the email.

We currently accept most insurance including Aetna, Asuris, Blue Cross Blue Shield, Cigna, First Choice Health, Humana, LifeWise, Premera, Regence, and United Healthcare. We also accept Medicaid (Apple Health) plans through United Healthcare and Wellpoint, as well as Medicare, CHAMPVA, and L&I.

We bill as a primary care provider. Please contact your insurance company to verify your benefits with us before scheduling an appointment.

If you get an error message when confirming your appointment, the most common reason is that the billing address you entered does not match what your bank has on file for your card. Please double check this.

Remember, if you need to be seen immediately, it is always acceptable to walk in.
