Lab Work: If you have not had labs drawn recently, except for simple sick visits, we likely need to check labs to help identify what's going on with you. If you have insurance, please include your insurance information. Indicate if you generally get labs at LabCorp or Quest. Alternatively, if you do not have insurance - no need to worry. We have connections with affordable cash pay labs!


Pre-Appointment Labs: If you'd like to complete lab work before your first visit for a more efficient consultation, mention this in your note. Schedule your appointment at least 1-2 weeks out to ensure results are available.


Mon-Fri 12-1 PM: Currently Telehealth only.
 

Initial Consults: In-office labs require in-person booking. Telehealth is fine if using LabCorp/Quest.


Deposit: A $40 deposit secures your appointment, and will be applied towards your visit cost. Please cancel 24 hours prior to avoid the fee.
 
 
Preparation: If you are able, please complete emailed electronic forms for consent and intake questionnaires before your appointment.
