5. Women's Hormone Replacement Therapy

Consult with provider to discuss evaluation for hormone replacement therapy. During this visit, your provider will discuss your personal & family history, current symptoms your are experiencing, and complete evaluation for potential hormone replacement therapy options.

*10-week hormone therapy plan cost is $298 and includes visit with provider, medication, and supplies (if applicable); you may use insurance coverage for lab testing or discounted self-pay lab available. (If determined to be a candidate, your consult fee of $99 will be credited to your plan cost)

Telehealth & In-Office visits available - please schedule based on your preference at time of booking.

6. Men's Testosterone Therapy

Consult with provider to discuss evaluation for testosterone replacement therapy. During this visit, your provider will discuss your personal & family history, current symptoms your are experiencing, and complete evaluation for potential testosterone replacement therapy options.

*10-week testosterone therapy plan cost is $298 and includes visit with provider, medication, and supplies; you may use insurance coverage for lab testing or discounted self-pay lab available. (If determined to be a candidate, your consult fee of $99 will be credited to your plan cost)

7. School & Sports Physicals

Comprehensive health examinations needed for school entry or participation in sports.

8. Thyroid Optimization

Evaluation and Management of Thyroid conditions.

9. Genetic Cancer Screening

The Genetic Cancer Screening Consult is a specialized medical appointment where a patient meets with one of our board certified providers to assess the risk for inherited cancers. During this visit, your provider will discuss your personal & family history of cancer to determine if genetic cancer screening is recommended to reduce your risk of developing cancer.
*This appointment can be completed via Telehealth or In-Office, please select appointment preference when booking.

B12 Injections
