Upon booking your initial evaluation you will receive a lab requisition form in your inbox. Please complete labs prior to attending your evaluation. You may use this requisition at any local lab should you wish to use your insurance for labs. Revolutionary Recovery LLC does not guarantee reimbursement by your insurance and is not responsible for any balance you may incur. We recommend cash paying for your labs through our lab store on Rupa Health as this is likely a cheaper alternative than insurance. To allow processing time please schedule your initial evaluation 7 days from when you will be getting your labs drawn. Labs must result before initial consultation.
