If you have Medicare/Medicaid, you must seek a Medicare/Medicaid provider for your services as we do not participate with those insurance plans. 

At the time of your appointment, you must be located in one of the states our provider is licensed. These states include: Hawaii or Utah. 

By scheduling an appointment, you are acknowledging Horizon Healing's US states they can provide mental health services and the non-refundable fees. 

Canceling appointments must be done with a 48-hour notice to avoid the late cancelation fee. Cancel your appointment within your patient portal. 

There is a non-refundable No-show and/or late cancelation fee of $50 that will be charged to an active credit card on file.
