A deposit of $50 is required to book starting September 1st. This will be credited to your account and taken off your bill at the time of checkout. If you do not cancel within 24 hours or no show the deposit is redeemed for late cancellation. 

Late Appointments: If you are 15 mins late for your appointment only $25 is credited to you bill, more than 15 mins late, the deposit is forfeited. 

Starting October 1st: All weight loss clients will need to submit insurance information. We accept the following insurances: BCBS, Humana, Cigna, Aetna, Anthem and United Healthcare. After your appointment, your provider will now send medications to the pharmacy to verify if insurance will cover the medication. If your insurance does not cover the cost of the weight loss medication, you have the option to purchase the compounded Semaglutide or Tirzepatide from us. If you chose to opt out of insurance a $99 provider fee is due prior to provider visit for both in-person and telehealth appointments.
