I would like to take a moment to explain our clinic's payment structure and policies to ensure you have a clear understanding from the outset.

*Please note: Due to this being a cash-based medical practice, we currently are not able to accept patients who are enrolled in Medicare/Medicaid benefits*

Payment Structure:
1. Initial Visit: The cost of your first visit includes time to discuss your symptoms, comprehensive medical history, and treatment options.

2. Monthly Membership Fee: As with the initial consultation fee, there will be an associated monthly membership fee. This fee allows you to access our services and schedule follow-up visits.

3. Visits and Additional Fees:
- With your monthly membership, you are entitled to one follow-up visit per month.
- If you choose to attend more than one visit in a month, there will be an additional fee of $75 per appointment.
4. All payments will be processed through “A Healthy-Eir Life Management Company LLC

Labs and Medications:
- Please note that laboratory costs and medication therapy are not included in all memberships. Some monthly membership fees, labs and medication therapies will be billed separately.

Appointment and Membership Policies:
1. Membership Cancellation: If you decide to cancel your membership, please do so at least 48 hours prior to the next payment date. If you do not cancel in time, the membership fee will be automatically processed.
2. Appointment Cancellation: If you need to cancel an appointment, you must do so at least 24 hours in advance. Failing to do so will result in a $50.00 late fee.

Consent for Treatment: As part of your treatment plan, you will need to sign a consent form acknowledging that you understand our approach may include off-label therapies, and not all therapeutic options are FDA approved. By signing, you accept the associated risks.
Compliance and Protocols:
- It is essential that all patients comply with obtaining required laboratory tests in a timely manner. Failure to do so may result in discharge from our practice.
- Any patient found to be taking more than the prescribed amount of medication will be immediately discharged.
- Patients who do not adhere to the organized treatment protocol may also be discharged from the practice.
